Appointment Information and Directions
Please be aware that the parking lot and entrance are located in back of our building. The parking lot is only accessible by an alley marked with an orange "Turn Here" sign. Once in the alley, we have a large sign marking our parking lot.
Because we at First Capitol Surgical Dermatology want to provide the best service possible, we ask that you give us 24 hours notice when canceling an appointment to avoid any office charges. It’s is important to note that consultation appointments are required before surgery. We're conveniently located at 901 South Fifth Street in St. Charles. We’re the all red brick building at the corner of McDonough and Fifth Street. McDonough is a one-way street, so enter our parking lot through the alley from Fifth Street.
Insurance Information
At First Capitol Surgical Dermatology, we provide quality care in a convenient manner. This includes participating in a wide range of insurance plans. Please contact our office to inquire about what plans we accept. Contact your insurance provider to inquire about what providers are in your network.
Due to federal, state and insurance company regulations we are no longer able to bill insurance companies for routine co-pays. We are subject to significant penalties if we do not collect your co-pay at the time of your visit. We request that you use cash, check, MasterCard, Discover or Visa for your co-pay at the time of your appointment. If your insurance company requires a referral, you must provide a valid physician’s referral at the time of your visit. If you need to cancel an appointment, please give us 24 hours advance notice to avoid being charged for the visit.
