Preparing for your Visit
Your surgery appointment may change after your consultation.
Please bring to each appointment:
Insurance card, photo ID, and co-pay if required.
Please arrive 15 minutes before your appointed time in order to fill out the required paperwork. If you are able to download and print the paperwork from this website, please do so, and bring the completed paperwork with you.
If your insurance requires a referral, please have your primary care physician fax it to us at 636-916-1561. Our address is: 901 South Fifth Street, St. Charles, MO 63301
Directions to the office: From Highway 70- exit Fifth Street South, travel approximately 0.5 miles. Across from Smoothie King just past Full Circle gun shop, turn left at “TURN HERE” orange sign into the alley. The parking lot will be on your right (marked First Capitol Dermatology). From First Capitol Drive-turn right on Fifth Street, then right into the alley just past the “TURN HERE” orange sign. The parking lot will be on your right (marked First Capitol Dermatology).
